Communications professionals can get a little nuts about mistakes like this because they can ruin relationships - the key concern of communications departments. Despite the origin of the problem - human or technical - if I were Blake Patterson, I would have phoned every person who received the multiple copies of the same message to apologize. In this case, I feel bad for both Blake and the recipient!
I am sure this is a mere technical problem - now this poor guy is prob going to have to deal with his boss bitching him out forever becuase this is published. I work for communications bosses - I know how nuts they get.
